DentoSim XR is a small public project on GitHub: an experimental dental-procedure training app for Apple Vision Pro, written in Swift for visionOS. The repository has 18 commits by three contributors made between October and December 2025, no releases, no tags and no licence, and no changes since 12 December 2025. What it does
- An immersive RealityKit scene with upper and lower dental arches, built from 17 USDZ models of permanent teeth (incisors to third molars, including a maxillary first molar with a cusp of Carabelli) and placed along a U-shaped arch.
- Four training modules defined in code: Basic Dental Examination (beginner), Class I Cavity Preparation (intermediate), Wisdom Tooth Extraction and Root Canal Access Opening (both advanced). Each is a list of steps with a tool, target area and tip.
- Eight virtual instruments: mirror, explorer, excavator, drill, forceps, elevator, syringe and scaler.
- Step checking when you tap a tooth: right tool, right tooth and right order. For example, forceps only work after the elevator has loosened the tooth. Wrong actions are logged as minor, moderate or critical errors.
- Visual feedback: a short pulse on success, a shake on an error and a changing material as work progresses, plus tooth states such as anesthetized, loosened, prepared and extracted.
- A score out of 100 at the end of a module, reduced by error penalties and by any time spent over the module’s estimated duration.
- An optional chat assistant that sends questions to the OpenAI GPT-4 API.
The README goes further than the code. It describes learning and exam modes where an instructor joins the session, realistic drill and suction sounds, detection of excessive pressure or a wrong working angle, and experience levels. We found no multiplayer, audio or force-sensing code in the repository. The interaction is a pinch or tap on a tooth that advances progress in fixed steps, not a tracked handpiece.
Who can use it and how
Study how a visionOS training app is structured
- Read the technical part of the README, which walks through the scene, tooth entities and interaction flow.
- Open ImmersiveView.swift and InteractionSystem.swift to see how taps are mapped to tools, tooth states and errors.
Prototype a new teaching module
- Add a module to ProcedureData.swift with its own steps, tools and target tooth (the code uses Universal tooth numbers, e.g. #18).
- Run it in the simulator and check the step order and the error messages with a clinical tutor.
Build and first run
- You need a Mac with a current Xcode that includes the visionOS SDK. Open DentoSim XR.xcodeproj, choose the Apple Vision Pro simulator or a paired device, and run. A device build needs your own Apple signing team.
- The chat assistant reads an API key from APIConfiguration.swift. Do not use the key string committed there. Create your own OpenAI key and keep it out of source control, as the file’s own warning says.
Limitations
- Prototype stage: 18 commits, no releases, no activity since December 2025.
- No licence, so you may read the code but have no granted right to reuse or redistribute it.
- Runs only on Apple Vision Pro hardware or the visionOS simulator.
- Simplified mechanics: no haptics and no force or angle measurement, and procedures advance by taps.

DentoSim XR vs alternatives
Established dental simulators used by schools, such as DentSim (Image Navigation), Simodont and SIMtoCARE, combine software with dedicated hardware, handpieces or haptics.
